Job Description:
Overview: Major Responsibilities: Responsible to ensure every
patient receives a thorough, comprehensive, and customized physical
therapy regimen, while receiving a unique experience. Provide a
written and hands on evaluation and immediate treatment of incoming
patient conditions to determine appropriate physical treatment
programs and consult with Clinic Director or referring physicians
regarding diagnosis. Responsible for ongoing scheduling of patients
to ensure fluid treatment progression. Develop and implement
Physical Therapy treatment programs based on each patient's
particular needs. Implement skills that were developed in clinical
experience during Physical Therapy doctoral/master's program.
Assist patients with all phases of Physical Therapy treatment by
using different techniques including exercise instruction (both
in-clinic and take-home instruction), manual therapy techniques,
modalities, activities of daily living, and functional or total
body progressions. Discuss treatment with patients and record
patient conditions and responses to treatments in patients' medical
record after each visit. Complete daily patient care notes,
progress notes, discharge notes and other required forms as
requested by treating physicians, rehab nurses or insurance
companies. Ensure daily treatment notes for all patients are
complete prior to end of day. Consult with Clinic Director or
referring physicians regarding patient diagnosis and communicate
changes via progress notes. Provide continuous communication with
referring physicians, Physical Therapy Assistants and Athletic
Trainers regarding patient therapy and equipment. Communicate with
Physical Therapy Assistants and Athletic Trainers regarding
re-evaluation findings to continually modify and update treatment
programs. Operate and apply knowledge of standard physical therapy
equipment. Complete weekly Caseload tool within assigned
timeframes. Accurately document treatments rendered in the
appropriate patient's medical record. Ensure that the billing
amounts on daily notes match treatment time. Responsibilities: Job
Requirements: Applicant must possess a Doctor of Physical Therapy
(DPT), or foreign equivalent. If the degree is obtained prior to
2015, then applicant must possess Master's degree in Physical
Therapy. Additionally, the applicant must have Physical Therapist
License in the state of employment or be eligible to obtain
Physical Therapist License prior to employment. Furthermore, the
applicant must have professional experience with: 1) Physical
Therapy techniques including exercise instruction (both in-clinic
and take-home instruction), manual therapy techniques, modalities,
activities of daily living, and functional or total body
progressions; 2) Preparing daily patient care notes, progress
notes, discharge notes and other required forms; 3) Operating and
applying knowledge of standard physical therapy equipment; and 4)
Handling a weekly Caseload tool or EMR. Qualifications: Applicants
must be licensed or license eligible in the state which they are
applying.
