Practitioner, youll contribute to our success in the following
ways:* Ensures that all activities performed align with the vision
of Elara Carings board of directors, executive team, and the
leadership of the Hospice team.* Prevents unwanted hospitalizations
by meeting patient and family needs in the home.* Prepares and
maintains accurate patient records, charts, and documents to
support sound medical practice and reimbursement for services
provided.* Compliance with applicable laws and regulations with
respect to Collaborative Agreements.* Completes accurate billing
and coding activities for all patient encounters according to CMS
guidelines and initiates reimbursement for medically necessary
visits in accordance with applicable law and regulation.* Provides
training and continuing education for staff.* Assists in
development of clinical practice guidelines/standards in support of
quality palliative care.* Adheres to the practice of
confidentiality regarding patients, families, staff and the
organization.* Collaborates/Communicates with Medical Director,
attending physician, branch leadership and IDG.* Performs other
duties/projects as assigned.What is Required?* Masters Degree in
Nursing and certification through a national Nurse Practitioner
Licensing board.* 3+ years clinical nursing experience* 1+ year
post-graduate clinical nursing experience in the hospice and
palliative care field.* Prior experience in Family Medicine,
Geriatrics and psychiatry preferred.* Current licensure in the
state of practice as a Nurse Practitioner and maintain such
licensure in accordance with applicable laws and regulations.*
Board Certification in Palliative Care Preferred.* Active DEA
license.* Able and willing to travel up to 50% as needed to support
business and patient needs.You will report to the Branch
