Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year of adult home health experience or 1 full year of hospice experience depending on the specialty. License: Must have license in hand. Certifications: CPR, BLS Must-Have: - Proficient with HomeCare HomeBase and OASIS. - Recent specialty experience required. - Must have a vehicle, valid driver's license, and auto insurance. - Comfortable traveling up to 75 miles for patient visits. Description: Provides coordination and direction of home nursing care. Care is based on physician orders and agency policies and procedures. Competent application of the nursing process is essential. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses. Conducts home visits and attends periodic office meetings. The average number of patients seen per day is 5-8. Contractor will be assigned between 5-10 patients per day. Must achieve approximately 30 productivity points per week. Types of cases include Start of Care, Wound Care, and Follow-up Visits. Must be open to traveling up to 75 miles from the assigned care center. Mileage is reimbursed at the IRS standard rate. Must accept all patients scheduled regardless of distance. All documentation must be completed within 24 hours of the appointment. No guaranteed hours with this contract; weekly productivity points must be met. On-call rotations may be required. Local candidates are accepted at the same rate.
