Must-Have: Comfortable with a case load of around 25 patients Acute
Inpatient Case Manager Description: Responsible for facilitating
the progression of care. Evaluating for the next appropriate level
of care. Conducting discharge planning. Working within an adult
inpatient unit. Possible case load may include med/surg,
